The Crash of Air Florida Flight 90 – Pilot Training and CRM Failures

Air Florida was a small intrastate carrier that expanded rapidly after the Airline Deregulation Act of 1978. Both pilots of Flight 90 were hired during this period
of rapid growth. For this case study, watch the video and review the NTSB report, focusing on both aircrew training and multiple CRM issues.
Prepare a 3-5 page paper (APA format, double-spaced, not including cover page) analyzing the crash of AF Flight 90. You will evaluate the performance of the
crew (and other teammates such as ATC, dispatch, etc.) with respect to this module’s lesson on Aircrew Training and CRM. Appraise the qualifications of the
crew and highlight any potential deficiencies. How might simulator training have been used to correct these deficiencies? What CRM failures can you
identify? Also, include a discussion of the principles of 6th Generation CRM/Threat and Error Management. Do not just summarize the events; include a
critical analysis!
